Vaccine Clinic at Ennis Care Center

Ennis Care Center will be hosting a Pfizer Covid Vaccine Clinic on Tuesday, April 27, 2021, from 10 a.m. to 6 p.m. The second dose will be on Monday, May 24, beginning at 10 a.m. A consent form is required before both clinics. Please email AGentry@capstonehc.com to schedule your vaccination, ask questions and acquire the consent form. Consent forms for the April 27 clinic (first dose) will need to be completed and submitted to AGentry@capstonehc.com no later than Thursday, April 22, 2021, at 2 p.m. Ennis Care Center is located at 1200 S. Hall Street, Ennis.
